Geriatric Care Management, Explained for Families

What geriatric care management and elder care navigation actually involve, what they do not include, and how to tell whether your family needs it.

Families searching for help with an aging parent run into a lot of names for the same kind of work: geriatric care management, elder care navigation, senior care coordination, care management, private patient advocacy. At Coastal Access Living we call it Family Advocacy & Care Navigation. The labels differ, but the core job is the same: one organized person keeps the whole picture so the family is not doing it alone.

What the work usually includes

  • Understanding the current situation: health changes, the home, the providers involved, and who in the family is deciding what.
  • A written plan the family can act on, with or without ongoing help.
  • Coordinating providers such as home health agencies, private-duty care companies, and equipment suppliers.
  • Preparing for and following up on appointments so questions get asked and answers get written down.
  • Helping with hospital and rehabilitation transitions, which is where things most often fall apart.
  • Organized updates to the family members the older adult has authorized.

What it is not

Care navigation is nonclinical. Coastal Access Living is not a home health agency, caregiver agency, nurse registry, or medical provider. We do not give medical, legal, or financial advice, and we do not provide hands-on care or transportation. Decisions stay with the older adult, the family, and the right licensed professionals. Our job is to make those decisions better informed and easier to carry out.
